Registration Information

Completed registrations can be dropped off at the Delaware Avenue School Main Office during office hours.

Applications must be completely filled out and signed to be considered valid.

Please review the DAS Cub Care Program Guide before submitting your application to ensure you are familiar with program expectations, procedures, and requirements.
